Hospital-Based Services

North East Specialized Geriatric Centre (NESGC) does not provide hospital-based services, but our team works closely in collaboration with hospitals and specialty services for older adults throughout Northeastern Ontario.

Health Sciences North

North East Specialized Geriatric Centre (NESGC) works closely with hospitals across Northeastern Ontario. In Sudbury, NESGC collaborates with Dr. McElhaney, at Health Sciences North, and the Consultation for Older Adults with Acutely Compromised Health (COACH) team. The COACH team works with frail seniors 65 years of age and older, who have been admitted to an inpatient bed, and who either have lost or are at risk of losing their independence in daily living.

Moreover, NESGC is proud to partner with the Short Term Assessment and Treatment (STAT) rehabilitation clinic. STAT operates an ambulatory rehabilitation clinic from Health Sciences North’s Outpatient Centre.

Sault Area Hospital

North East Specialized Geriatric Centre has worked closely with the Sault Area Hospital (SAH) and the North East Local Health Integration Network to develop and launch the Algoma Geriatric Clinic. The Algoma Geriatric Clinic now operates as a service of the SAH, and as a partner of NESGC. Individuals in the Algoma region wishing to be seen at the Algoma Geriatric Clinic must be referred by a physician.

Senior Friendly Hospital

From the Senior Friendly Hospitals website: The Ontario Senior Friendly Hospital framework provides an evidence based blueprint to guide hospital-wide improvements in services for frail older adults. The framework comprises five inter-linked components: organizational support, processes of care, emotional and behavioural environment, ethics in clinical care and research, and physical environment. Better outcomes for frail older adults emerge when these components are considered together.

The vision of the Senior Friendly Hospital Framework and Toolkit is to optimize the health and well-being of frail and at-risk older adults when they are in hospital and their safe transitions home.

North East Specialized Geriatric Centre is proud to contribute to the Senior Friendly Hospitals initiative by having our Executive Director participate on the Regional Steering Committee for the North East Local Health Integration Network, as well as the provincial Senior Friendly Hospital Leads Committee.

Geriatric Emergency Management (GEM)

Geriatric Emergency Management (GEM) is a program that provides special assistance to older adults accessing hospital emergency departments. The Regional Geriatric Programs (RGPs) of Ontario have been instrumental in advancing the development of the GEM program and the placement of GEM Nurses in emergency departments throughout Ontario. In Northeastern Ontario, GEM Nurses are currently in place in hospitals in North Bay, Parry Sound, Sault Ste. Marie, and Sudbury.

More information, resources, and the GEM Toolkit can be found on the GEM website.
