2009: NESGC was launched by the City of Greater Sudbury.
2019: Located at the Pioneer Manor Campus in Sudbury, the program initiated an amulatory clinic in Sudbury, and outreach clinics in the regions of Algoma, Cochrane, Nipissing, and Parry Sound.
2012: The clinical leadership team expanded adding a new care of the elderly physician.
2012: North Bay Regional Health Centre secured program sponsorship through a competitive bidding process.
2014: With the support of the North Bay Regional Health Centre and the North East Local Health Integration Network (NE LHIN), NESGC has expanded its programs and services to include outreach clinics in Timiskaming and in communities along the James Bay Coast.
2014: NESGC transitions from being hosted by the City of Greater Sudbury to Health Sciences North, in Sudbury.
2016: Expansion to the Cochrane District partnership with the Timmins Academic Family Health Team
2016: Clinical leadership continues to grow with the addition of a new care of the Elderly Physician.
2016: NESGC's sponsorship transitions from North Bay Regional Health Centre to Health Sciences North in Sudbury.
2017: NESGC further develops adding two new services: Geriatric Outpatient Rehabilitation Service located at Sudbury Outpatient Centre and Geriatric Inpatient Consultation Service located at Ramsey Lk. Health Centre in Sudbury.
2018: Geriatric Mental Health Outreach Service and Behavioural Supports Outreach Service transfer from the HSN's Mental Health and Addictions program to NESGC.
2020: NESGC launches a new Coordinated Access model with a one referral pathway for all services in the Sudbury region, ensuring access to services, eliminate duplication, reduce wait times, and enhance service navigation.
2021: NESGC launches a new inpatient program, the Reactivation Care Unit, located at the Daffodil Lodge at the Ramsey Lk. Health Centre.
2021: The RCU, a 20 bed unit is set to expand to a 52 bed Reactivation Care Centre which will be housed in the former Children’s Treatment Centre, set to transition in 2023.
2021: Development of the Geriatric Medicine Urgent Response Team.
