Service Description
Specialized team members provide comprehensive assessment, treatment planning, counselling, therapeutic intervention, health teaching, education, advocacy, consultation and collaboration in order to support older adults with complex mental health needs.
Target Population
Older adults, 65 years of age and over, with complex needs associated with late onset mood disorders (e.g. depression, mania) anxiety, psychosis, progressive cognitive disorder, social isolation, fear of self-harm, substance misuse and addictions; mental health complications associated with neurodegenerative cognitive impairments (e.g. Alzheimer's disease); worsening of chronic mental illness (e.g. schizophrenia, bipolar disorder, etc.) in the context of aging and geriatric complexity.
